This is an example of a form used in one of our primary schools to log low-level child safeguarding concerns. Adapt's simple to use analysis tools can readily flag up trends and patterns in the concerns, possibly indicating more serious issues at home.
Example of a parent contact form
This is an example of a form used in one of our primary schools to log contact with parents. Adapt allows past entries to be grouped by parent, date, child etc. simply and quickly. This allows head teachers to view all contact with any particular parent at the click of a button.
